Back pain in children
is abnormal and should be investigated thoroughly. There are a large
variety of causes for back pain, ranging from benign musculoskeletal
soreness to more sinister causes such as spinal cord tumors. Luckily,
the vast majority of back pain in children is posttraumatic in nature,
and is due to musculoskeletal strain. This type of pain is usually self-limited,
and can be treated with conservative measures such as Tylenol, Motrin,
and rest. Back pain that is not self-limiting, and persists beyond two
to three weeks should probably be investigated with at least plain films
and probably other imaging studies.
Causes of back
pain under this heading include manifestations of congenital bony anomalies,
tumors of the surrounding soft tissue of the spine, tumors of the bone,
or tumors within and around the spinal cord. In addition, vascular malformations
of the spine and spinal cord can also be involved. Infections are also
a significant possibility, and the leading type of infection in children
causing back pain is discitis. This usually results in antibiotic therapy
and rest. However, the main point is that back pain that is not self
limited and persists longer than approximately two weeks should be investigated
by a qualified individual, preferably one with experience in dealing
with pediatric spinal disorders.
